Fee Schedule 2023/2024 (applies to fees payable relating to the period on and after 1 April 2023)
Refer to the current Fee Schedule for the various fees issued by the Board.
PLEASE NOTE:
ALL application forms are to be submitted to the Board via post as hard copy documents.
Only APC applications and Non-Practising Declarations are done online – these do not require any hard copy forms.
Please DO NOT submit any original documentation with an application, only correctly certified copies.
Application Forms & Requests for Information
| New Zealand Graduate Registration | Use this form if you completed a New Zealand Dietetic Qualification. A Criminal Conviction Check (Request for your own criminal conviction history check) must be completed and accompany your application. |
| Recognition of Overseas Qualifications | Use this form if you trained overseas and wish to have your dietetic qualifications recognised in NZ. Please refer to the checklist to assist you in completing the ROQ application. |
| Overseas Trained Registration | Use this form if you trained overseas and have now passed the Boards Registration Examinations. A Criminal Conviction Check (Request for your own criminal conviction history check) must be completed and accompany your application if you have lived in New Zealand for 12 months or more. |
| APD Mutual Recognition Registration | A Criminal Conviction Check (Request for your own criminal conviction history check) must be completed and accompany your application if you have lived in New Zealand for 12 months or more. APD applicants must successfully complete the MauriOra Foundation Cultural Competence Course and a copy of the certificate must accompany your application. |
| Board Registration Oral Examination | This form is currently unavailable. Please contact the Board for further information if required. Use this form if you have had your overseas qualifications recognised and passed the written examination. Applicants must successfully complete the MauriOra Foundation Cultural Competence Course and a copy of the certificate must accompany your application. |
| APC Application | This is completed online through the Dietitians Portal. |
| Return to Practice Application | Use this form if you are returning to practice after a break of more than 2 years or last held an APC more than 2 years ago. You may be required to complete a Criminal Conviction Check (Request for your own criminal conviction history check) as part of your application. |
| Request to cancel entry on Register | Use this form if you wish to cancel your entry on the Register. Please read the Removal from Dietitians Register Process for more information. |
| Restoration to the Register (s142) | If your entry has been cancelled under s142 of the HPCA Act (2003) use this form if you wish to restore your name to the Register. A Criminal Conviction Check (Request for your own criminal conviction history check) must be completed and accompany your application. |
| Restoration to the Register (s144) | If your entry has been cancelled under s144 of the HPCA Act (2003) you need to use this form if you wish to restore your name to the Register. A Criminal Conviction Check (Request for your own criminal conviction history check) must be completed and accompany your application. |
| Request for copy of Register or other data | Please contact the Registrar if you would like a copy of the Register or other data for research purposes – there may be an associated cost. |
